Proactively, with a smile: "No worries — psoriasis, not contagious. I know it can look a little odd sometimes." Then shake hands as usual.

If you want to get ahead of it before the meeting: "Quick heads-up: I'm having a psoriasis flare-up right now, looks a bit noticeable. Not a big deal for me — just wanted to mention it briefly."
